Username: [My username]

Type: Mysql
Servername: Localhost (99.9% of the time it will be localhost)
Port: 21
Username: Is your sub-domain name. Eg, www.domain.vndv.com, your username would be domain
Password: Your Password for web hosting
Jacob.

Incorrect - your username and password are the user and associated password you set up with the database, so domain_something is your username.

and the port number is DEFINITELY not 21. 21 is used for FTP, so I'm assuming that NaRzY isn't retarted and was just thinking of ftp and not mysq.
Normally if you leave the mySQL port empty it will try to use the default.
