I am unable to access PHP My Admin Alternative due to login information

my db name is quantumplanets_logoffkillaccount or logoffkillaccount
my password is ********

also the directions for my account is Only lowercase English alphabet letters, digits and $ allowed with a maximum of 53 characters... but i heard your username has to be 16 characters or less. so im confused about that one
so does my user name equal to quantumplanets_uuuq_com_b 25 characters
or quantumplanets_b 16 characters
or b 1 character
or is it a user name that's not listed

Also i am unable to log into phpmyadmin directly, the following message comes up when i click the link
