Ok before anyone tells me to go to the tutorials and look for the member registration tutorial I will tell you that is not what I would wish to do.

I wish to create a Registration form for a hockey tournament which a coach may enter:

Team Name
Team Registration Number
1 or 2 goalie spots
Team Roster (if possible with numbers of players)
Coaches/Trainers/Staff/Manager and their registration numbers
Managers E-Mail

with the info that the manager/Coach enters into the form i wish for it to be out put like the example below:

Neptune Saucers 6194
10 George Alpha
2 Albert Bravo
6 Eric Alpha
17 John Bravo

Coach John Smith Certification #
Trainer Mary Smith Certification #
Staff Joe Smith Certification #
Staff George Smith Certification #
Staff King Smith Certification #


and the managers email showed on there aswell just not in the same list.

I would also like it to out put it into a spread sheet if possible. I must be able to see all info input into this form. I do not wish for the user to sign up for my website just submit info for the tournament my Hockey organization is hosting.
