I had two thing in there, different index page name and 404 error, but it stopped working now.

What do you mean by it "stopped working"? Are you getting an error when you try to access your site? Or is it not doing what it is supposed to?

Please post here the contents of your .htaccess file along with any errors you get. Be sure to check your error_log files in all of your directories to make sure that no errors are occuring behind the scenes that you might not be aware of.
